Practice Policies
Please Review Carefully:
- The practice utilizes the IQ Health Patient Portal as the primary method of communication for non-urgent matters.
- Prescription refill requests are not accepted after office hours.
- Medication refills should be requested during office visits. Patients are responsible for ensuring they have an adequate supply of medication until their next scheduled appointment. Prescription refill requests must be submitted through the Patient Portal and allow the office 24 business hours for processing.
- Controlled substance medications are not prescribed during the initial visit. Medical records and required testing must be received and reviewed prior to consideration of any controlled substance prescription. If prescribed, patients must sign and comply with the Controlled Substance Agreement.
- Patients must respond to appointment notifications to confirm scheduled appointments. Unconfirmed appointments will be automatically canceled.
- Patients are expected to arrive at least 15 minutes prior to their scheduled appointment time. Late arrivals may be asked to reschedule or wait until scheduled patients have been seen.
- Patients who fail to appear for a scheduled appointment without providing proper notice may be charged a no-show fee and/or may be dismissed from the practice. New patients who do not appear for their scheduled appointment may not be offered another appointment.
- All copay and outstanding account balances are due at the time of service.
- Completion of forms (including FMLA, disability, and similar documents) requires an office visit and a $25 form completion fee, which must be paid prior to processing.
- The practice follows a policy regarding diagnostic tests and laboratory results. United Medical Associates will notify patients of results within 14 days (approximately two weeks) via the Patient Portal or by phone. If patients do not receive notification within this timeframe, they are responsible for contacting the office. Patients should NOT assume that no news is good news.
- A wellness visit is not the same as an office visit. Any medical concerns addressed outside of a wellness or preventive screening visit may result in additional copays and/or deductibles based on the patient’s insurance plan.
- United Medical Associates does not evaluate or treat work-related injuries or injuries related to motor vehicle accidents.
- The practice utilizes artificial intelligence (AI)–assisted technology to support clinical documentation and medical record charting. This tool is used to enhance accuracy and efficiency and does not replace the provider’s clinical judgment. All documentation is reviewed and approved by the provider.
- Patients are expected to treat all staff, providers, and other patients with respect and courtesy. Disruptive, abusive, or disrespectful behavior toward staff or providers will not be tolerated and may result in termination of the patient-provider relationship. The practice reserves the right to take appropriate action to ensure a safe and respectful environment for all patients and staff.
